Addison Cut off saw motor

I have a cut off saw, but the rotary switch broke whilst in transit.

Here is the plate from the motor. There are 6 connections on it U1/V1/W1 U2/V2/W2



Can anyone tell me which of these starters and 2 speed switch would be suitable?
