Have a look at http://e3d-online.com/Kraken-Supplies/The-Kraken - a bit expensive (but no more than three hot ends) and water cooled so ok in a heated chamber.

I don't think you do - if by rapid you mean G0 commands, the speed is set by the machine controller.

You want the head about half the slice thickness off the table (I'm assuming sli3er has used 0 for the first layer height) - this squashes the first layer down firmly.

I used clean glass at first and it was a bit hit and miss - I would think the frosting is an improvement.
The PVA is quite dilute (5:1), applied with a tissue and will dry while the bed heats up.... -

You can get USB to SATA adaptors which let you attach a bare hard drive as an external drive. I've used them for data recovery before - very handy for cloning a laptop drive.
